(231) 672-3364 The Muskegon Community Health Project (MCHP), a non-profit community health network, is nationally recognized for its pioneering efforts in leading collaborative community efforts to address access to health care; and improve the health status for low-income, underserved, and uninsured communities. MCHP is currently supported by Mercy Health Partners and Trinity Health as a pilot site for innovative approaches to creating access to the uninsured and underserved populations through its Community Benefit Office. MCHP has a racial and culturally diverse staff of 26 full-time and one half-time employees who are actively engaged in community health partnerships and capacity building programs. MCHP serves a geographically diverse 3-county area with a number of programs. Through its relationship with the Trinity Health and Mercy Health Muskegon, MCHP’s staff link low-income and other vulnerable populations including the African American and Hispanic communities, as well as their health providers with community-based services. MCHP uses a variety of outreach strategies to distribute information, identify people eligible for programs and get them enrolled. Specifically, MCHP’s work falls within three core areas: health resource partnerships; community education; and community wide coverage/access. The Health Project pioneers four "Pathways" projects using the acclaimed Pathways model developed by Drs. Sarah and Mark Redding, MD. Pathways is a community care coordination and accountability model that uses a Community Hub and Community Health Workers to ensure that low income and vulnerable populations are connected to the health and human services needed to improve health outcomes. Our service programs include outreach and enrollment for families needing Medicaid, SCHIP, ACA Marketplace, Food Stamps, and other local health services. In addition, MCHP provides screening and education programs for diabetes, prostate cancer, pulmonary disease, and annually navigates hundreds of uninsured individuals to primary care homes and health coverage. MCHP also provides assistance for those seeking specialty services such as diabetes retinopathy exams, vision and hearing services. MCHP has run a pharmaceutical assistance program since 2003 to address the medication needs of low income, uninsured and underinsured individuals.
